How to file a petition to the Commissioner?

I. Petitions to the Ukrainian Parliament Commissioner for Human Rights shall be submitted in writing

It shall be mentioned in the petition:

1. First name, patronymic name and last name

2. Postal code and notification address

3. Name of the state or local authority or their officials that violated your rights

4. The essence of your rights’ violation

5. When and for what period have your rights been violated?

6. Measures you have undertaken for restoration of your rights. Authorities or officials you have applied to (please, attach copies of their replies)

7. Have you filed a complaint in court to protect your violated rights?

8. The essence of your request or petition

9. Date and manual signature

10. In case of filing a petition on behalf of the third party copies of documents that certify powers to act for it shall be provided.

Petitions to the Ukrainian Parliament Commissioner for Human Rights shall be submitted in writing to the postal address as follows: 21/8, Instytutska str., Kyiv, 01008, Ms. Valeriya Lutkovska.

II. Petitions to the Ukrainian Parliament Commissioner for Human Rights can be submitted by e-mail at: hotline@ombudsman.gov.ua.

Attention! Petitions submitted by e-mail shall comply with the rules on the submission of written petitions.

III. Petitions to the Ukrainian Parliament Commissioner for Human Rights shall be received in writing during reception hours at the Community liaison office of the Ukrainian Parliament Commissioner for Human Rights.

Working hours of Community liaison office of the Ukrainian Parliament Commissioner for Human Rights:

Monday-Thursday 9.30-13.00 – 14.00-17.30

Friday 9.30-13.00 – 14.00-16.00

Saturday-Sunday – days off
